Open account in any brokerage. I use fidelity as it's super easy interface. Creating a rollover account is less than 5 minutes work

If fidelity has your old 401k company, you can do direct transfer. Otherwise, they will give instructions to send payments. Call your old 401k provider, and ask them to send check to Fidelity with account number. This works

One important point. your 401k account will have option of sending check to you. Never do this, as it'll get treated as 401k distribution and IRS will send tax bill to you after 2 years. If you forget, you will end up paying double taxes
