Kindly collect this copy of letter back as acknowledgement towards submission of your documents. Documents to be submitted: 1. Applicant’s valid passport with all previous passports. 2. A completed DS-160 application confirmation page with barcode. 3. Photograph with white background (2 x 2 inches (51 x 51 mm) in size. 4. Supporting documentation in ORIGINAL and one photocopy required for your visa type. (Documents as applicable: I-797, I-129, I-20, DS-2019, CDC if issued) 5.
