In the United Kingdom, lifestyle and health magazines are abuzz with the news of the arrival of a 60 milligram pill — Alli (pronounced AL-eye). The first over-the-counter anti-obesity pill, Alli blocks out the fat in our food, about 25 per cent of it, doesn’t allow it to be absorbed and throws it out, quite literally. Medically-speaking, Alli contains the drug orlistat which blocks the enzyme lipase in the intestine. The result: Body doesn’t absorb all the fat in the food and is forced to use stored fat.
According to estimates, for every four kgs lost by exercising and following a healthy diet, those on Alli will lose six kgs — that’s an incredible 50 per cent more. If target weight loss is 16 kgs, at the rate of dropping four kgs a month it will take about four months to hit the target with diet and exercising. Add Alli to your weight loss programme and you could be dancing on the weighing scales in just two-and-a-half months.
Alli has been approved by the US’ Food and Drug Administration and has been in use in the US for over a year.
Indians are genetically susceptible to weight gain and, according to National Family Health Survey, already 12 per cent of our men and 16 per cent women are obese. According to Union health minister Dr Anbumani Ramadoss, “India appears to be overtaking countries like the US in obesity”. Dr Ramadoss has been worried about India getting obese and that’s why, March 19 onwards, it will be mandatory for all packaged food articles to carry its nutritional value, including fat content and calories.
So is India ready for Alli? In girth and weight it is, scream doctors. While Alli manufacturer GlaxoSmithKline won’t commit when Alli will be available in India, the fact is that Alli’s various avatars — larger, on-prescription-only pills — are already available in India and have been doing rather well. Orlistat brand names include Olistat, Vyfat, Obelit, Reeshape etc. All these pills, and Alli, are to be taken thrice a day immediately before or up to one hour after the three main meals.
Dr Dheeraj Kapoor, senior endocrinologist, Artemis Hospital, Gurgaon, said, “I have been prescribing orlistat for patients with a BMI of 33-34 and the response has been very good. I have a patient who lost 5.4 kgs in a month. Without orlistat, the weight loss would have been around 2.5 kgs”.
Most doctors love orlistat because, they say, it doesn’t have any medical side-effects. “The side-effects are mostly social — flatulence and passing stool, without your knowledge or permission. These can be embarrassing, but are not harmful”, says Dr Kapoor. These too can be minimised by taking a low-fat diet. Other orlistat must-do’s include: taking vitamin supplements and committing to a low-fat, low-calorie diet, and regular exercise.
Apart from that, doctors say, they’d be happy for Alli to be sold in India and are not stressed about people self-medicating.
“Most anti-obesity drugs go to the brain to suppress appetite. Orlistat has no effect on the central nervous system like causing anxiety or depression. This is one drug that doesn’t go into the system, but works locally in the intestine. There is no effect on the blood pressure and, in fact, studies have shown orlistat to be the only drug to suppress diabetes. If a patient has had a heart attack or suffers from diabetes, orlistat is preferable”, says Dr Anoop Misra, director and head of the department of diabetes, obesity and metabolic diseases, Fortis Hospital, Noida.
Yet another advantage of orlistat is that it is the only drug that can be given to obese adolescents. “No other drugs can be used for obese teenagers as they can have long-term as well as short-term effects, but orlistat has manageable side-effects. If Alli comes to India, it will go for over-the-counter sales, and sales will skyrocket. Since the dosage is just 60 mg, it will not have any side-effects”, Dr Misra said.
Dr Shashank Joshi, endocrinologist, Lilawati Hospital, Mumbai, said, “Indian diet is rich in carbohydrates and fats. Orlistat has this unique property of being a fat blocker. It is a smart pill, and a very, very safe one. Orlistat doesn’t work on the brain — it is not an appetite suppressor. The drug of choice for obesity is no drug at all lifestyle and behavioural changes. If at all a drug has to be prescribed, safety is a major factor. Drugs like sibutramine can cause neuro-psychological disturbances like memory loss. One drug which is safe is orlistat”.
