Many expressed an interest in purchasing life insurance in addition to the $50,000 provided by the Company. In response, we are offering a voluntary benefit through Prudential. You can purchase up to $300,000 in life insurance for yourself and $20,000 for your spouse with no medical questions asked (Prudential reserves the right to request medical information if we do not have 20% of our employees participating in the plan – Prudential will contact you directly if this information is required). More coverage is available with a completed health statement questionnaire. Your premiums can be paid through convenient payroll deductions.

Voluntary Group Universal Life (GUL) and Dependent Term Life insurance are now available to all associates on US payroll. These life insurance products, issued by the Prudential Insurance Company of America, let you purchase additional life insurance at group rates.

Enrollment is open to you through Friday, August 3, 2007. The coverage(s) you select will go into effect September 1, 2007.



Ø What is group universal life (GUL)?

o GUL offers you the opportunity to purchase affordable life insurance protection and build cash value by making additional contributions to a cash accumulation fund that earns tax-deferred interest at competitive rates. You can make those additional contributions either through payroll deductions or by lump sum deposits.



Ø How much coverage can I purchase for myself?

o Coverage is available in increments of $25,000 up to $1,000,000, not to exceed five times your covered annual earnings.

o You can select up to $300,000 without providing evidence of insurability satisfactory to The Prudential Insurance Company of America. If you select a coverage amount that exceeds the guaranteed coverage amount ($300,000), you must complete a Health Statement Questionnaire. Prudential will reserve the right to request Evidence of Insurability from all participants if we do not meet participation limits. More information will follow on this.



Ø Is coverage available for my spouse and/or children?

o SPOUSES

§ If you have enrolled for Group Universal Life (GUL) coverage you may enroll your dependents for coverage.

§ You may select to enroll your spouse for up to $20,000 without providing evidence of insurability satisfactory to The Prudential Insurance Company of America, if you enroll your spouse during the open enrollment period.

§ Coverage is available for your spouse in increments of $10,000 to $100,000, not to exceed 100% of your GUL coverage amount.

§ If your spouse is confined for medical care or treatment at home or elsewhere, coverage will begin when confinement ends.

§ Coverage will end on your termination of employment or as specified in the plan booklet. Insurance may be converted to an individual life insurance policy issued by The Prudential Insurance Company of America.



o CHILDREN

§ Coverage is available for all your children 14 days to age 19 for $10,000.

§ Coverage begins at 14 days and continues to age 19, if unmarried. If the child is unmarried, dependent on you and a full-time student, coverage continues to age 25.

§ Your children include your legally adopted children, stepchildren and foster children who depend on you for support.

§ No evidence of insurability satisfactory to The Prudential Insurance Company of America is required.

§ If your dependent children are confined for medical care or treatment at home or elsewhere, coverage will begin when confinement ends.



Ø How much does this benefit cost?

o Cost is dependent on the coverage amount elected and your current age. For specific rate information, please log into the Prudential website (see How Do I Enroll below).



Ø Can I continue this coverage if my employment with Cognizant ends?

o Prudential offers conversion and portability options to participants upon termination of employment. Eligibility for these options will require continued residence within the US.



Ø When can I enroll in this benefit?

o The enrollment period for this new benefit will be open to all US associates from July 5 through July 27, 2007. Coverage will go into effect September 1, 2007.

o If you do not enroll at this time, you may enroll during a future open enrollment period. Open enrollment is typically held in November/December for a January 1, 2008 effective date. Open enrollment will be formally announced.



Ø How do I enroll?

o ENROLLING ONLINE

§ Access the enrollment website at https://www.prudential.com/giwebenroll.

§ Read and follow the instructions carefully.

§ When prompted for your user id use your social security number (do not use dashes).

§ When prompted for your personal Identification number (PIN), use your date of birth (MMDDYYYY).

§ Make your selections and review them for accuracy.

§ Be sure to click CONTINUE for each of your coverage selections.

§ Review your coverage selections for accuracy and print a copy of your confirmation statement.



o ENROLLING BY PHONE

§ Call 1-866-2SIGNUP (274-4687), toll free

§ Listen carefully and follow the step by step instructions

§ When prompted for your user id use the last four digits of your social security number.

§ When prompted for your personal Identification number (PIN), use your date of birth (MMDDYYYY).

§ Make your selections and review them for accuracy.

§ Submit your selections.



Ø What do I do if I am experiencing difficulty with the Prudential website?

o You can print an enrollment kit (attached). The enrollment forms can be faxed or mailed directly to Prudential.
